If you meant game ratings, he was better in 3 ratings, you were equal at 1, and you were better at 2?

First let me say, that the game ratings DO NOT reflect everything the GE takes into account when simulating a game.
Many importants details that could eventually decide over win or loss, are NOT reflected in the game ratings.
For example things like form of your players, motivation of your team, etc... are just a few things that also play a part in the bigger picture.
That's the reason, that regularly you will see teams win games against other teams who have better ratings.

But that aside, it seems to me the ratings of both teams are about 50-50

At end of the game, with 0 or 1 second remaining, my best player on the team (PG) fouls the other team. They hit 2 free throws, end of game.

I know it happens, but this is annoying as hell. Not supposed to happen, especially since the other guy was not shooting.

How are effort and form (GS I assume) not reflected in the ratings? More effort boosts some of the ratings, mainly defense, the same way home court does. Form boosts the way individual skills contribute to the team's ratings. So both are reflected.

There is a suspicion that the current team and individual ratings do not reflect correctly the GE, ie. some skills are undervalued in the ratings (SB being the obvious candidate here since its impact has been boosted) and others are overvalued. In particular shooting ratings by position are often mind boggling compared to the actual results. I have a specific suspicion that shooting ratings you see in the box score are not calculated properly when zone defenses are involved (the defenders in this case are not fixed) or perhaps even when defensive switches are used.

@ lemonshine
there is also things like experience of your players amongst others,... which is not reflected into ratings

@ trainerman
I agree with you,

thus, my advice try to reduce your turnovers by training more on passing first and ball handling second,

espercially on your PG and SG.

This way, you will reduce your turnovers and strengthen your offensive flow, which also means : you will have more ball possession and more importantly win more games...
